Please submit sign up sheets here, using the following template, if you wish to be apart of the RP, Star Trek: Into Darkness. If the information provided in the template is vague or requires a better understanding, please ask in the Kerkovistan chat for help.

Faction: (In this section you can choose one of the three factions, United Federation of Planets, the Cardassian Union, and the Romulan Star Empire)
Ship: (Please choose a ship from the faction you have chosen from the aforementioned three, Federation Vessels, Cardassian Vessels, Romulan Vessels, please only use 24th century vessels)
Ship Name: (Please choose a name for your ship that fits with the faction you have chosen(Name Generator))
Ship Configuration: (Some ships could be reconfigured for different missions, if your ship can be configured for different missions, what configuration is your vessel?)
Ship Complement: (State your ship's crew and passenger complement)
Captain: (Please state the name of your ship's captain(Name Generator))
Captain Species/Gender/Age: (Some basic information on your captain, if you choose Cardassian or Romulan, do remember to follow their guidelines on positions of power in high ranking positions(Romulans were xenophobic, and Cardassians were sexist))
Other Crew (Rank/Name/Position): (This section is entirely optional, if you wish to put the names of your command crew, or whatever crew members you wish to name, please do so here)
